Vendor application for registration, 1957.

These forms were used by vendors to be placed on the state's mailing list so they could receive specifications and invitations to bid on items and commodities they produced or provided. The application consists of a cover sheet that the vendor completed indicating his/her address and signatures which would appear on bids and a brief description of his/her business. The remaining pages of the form consist of a list of commodities, which the vendor indicated by an (x) next to the commodity, which he produced or sold and would like to bid upon.
